Who We Help
This program is for Medica members who:
- Are 18 or older
- Live in Hennepin, Ramsey, or Anoka counties
- Have health challenges that make it difficult to manage medical care
- Could benefit from support with food, housing, transportation, and other needs
What do Community Health Workers do?
Community Health Workers (CHWs) are trusted helpers who often share common experiences or backgrounds with the people they serve. They work beyond hospital and clinic walls, meeting you where you are and finding creative ways to connect you with the resources you need.
From helping you apply for benefits to joining you for exercise at the Y or making sure you get to important medical appointments, CHWs focus on your goals and address the root causes of health challenges, including social and everyday factors that affect your wellbeing.

We reach out to eligible Medica members directly because we believe we can help make a positive difference in their health and wellbeing. We cannot email or text you due to privacy laws (HIPAA), so phone calls are our main way to connect.
When you speak with us, our Community Health Workers can:

- Help you get started with services
